2. Provide necessary documents
   1. Child’s Sacramental Certificates: Baptism, First Eucharist (each child) If your child received sacraments at Holy Trinity, we must know the year and date in order to verify in the Sacramental Register.

   3. Proof of Previous Formation (if coming from another Parish)—this can be: a letter from the other parish’s DRE or Pastor with the dates and formation attended, or a letter from a Catholic School with the dates and formation attended.
3. Schedule an Inquiry Meeting with the Director of Faith Formation or a Catechist.
   1. This meeting helps us to see where a Confirmand is on their faith journey and what kind of help and formation they need.
4. Attend all Confirmation Preparation Sessions.
5. Schedule a Discernment Meeting from 6-2 weeks before Confirmation Mass.
   1. This is required to discern the desire, readiness, and preparedness of a Confirmand to receive Confirmation. No discernment meeting means that an individual is assumed not ready or not willing to receive the sacrament at this time.
6. Attend Confirmation rehearsal and the Confirmation Mass.

If you are a parishioner of Holy Trinity, and desire to have your child confirmed at another parish, then you will need a Letter of Good Standing and/or a Letter of Permission from the Pastor.

If the parish is within the Diocese of Fort Worth, you must:

• Verify your sacraments received (Baptism, Confirmation, Eucharist, Marriage)
• Verify that you are a registered member of Holy Trinity.
• Submit the following to generate the letter:
  • Full Name and Address of the Church
  • Name of the Pastor of the Church
  • Name of the celebrant of the sacrament(s), if different from the pastor.
  • Name of the Child, and the Child’s parents.
• Note, requesting permission does not guarantee that our Pastor will grant it, nor does it guarantee the Pastor of the church in question will grant it.

• It is your (not the parish’s) responsibility to receive permission from both the Diocese and parish where the sacraments will take place, and permission from the Diocese of Fort Worth and the Pastor of Holy Trinity.
• Note, requesting permission does not guarantee that it will be granted.

• Must complete the Sponsor Form

• Must be 16 years or older, in good standing with the Church, and a proper example of Christian life.
• Must be Sacramentally Initiated (Baptism, Confirmation, Eucharist)
• If married, must be in a valid, Catholic marriage.
• Must verify their sacraments by submitting all sacramental certificates.
• If from another parish, must verify their good standing with a letter, signed by the pastor.
• There is only one (1) sponsor for Confirmation.
• Parents may not be sponsors.
• Non-Catholics may not be sponsors, but may witness.
